In our aromatherapy certification online course, we require students to complete ten case studies. This will help the student to gain understanding in how to perform an aromatherapy client intake and what duties the aromatherapist will perform as a certified aromatherapist.
Case Study Documentation
We ask each student to use the Client Intake Form and the Client Intake Notes for their documentation. As an aromatherapist, you will ask each client to fill in the intake form or you can interview the client and write the information for them. After you have filled in the form with all the data necessary and have gathered information about their health history, you will be using the knowledge you have gained from the aromatherapy certification course, as well as your intuition in determining the most chronic complaint that the client is dealing with.
Identifying Complaints and Which Essential Oils to Use
Some aromatherapists will use the Therapeutic Blending With Essential Oil book you can look up that complaint and see which oils are the most beneficial for it. If the condition is not listed, you may find several complaints related to it, such as stress, skin issues, poor circulation, etc. that are close and/or related to the particular condition you are working on alleviating. When multiple conditions are an issue, you will look for essential oils that are common for all complaints. Only one essential oil will be chosen from each of the columns (top, middle base notes) for the therapeutic blend you are making.
Method of Delivery to Use for Essential Oils
The type of application of the product can vary, such as room spray, diffuser blend, massage oil, body lotion, bath salts, etc. depending on how you determine its best use. In some cases, you may want to make the blend in various forms so the client has a choice and multiple methods of application. For instance, you may want to create a room spray as well as a body lotion for topical use.
Fill out the assessment for each client you formulate blends for. This will help you in determining the best choices for the complaints that have and any essential oil precautions you need to be aware of when making their blend(s).

Be sure to take into consideration the many factors such as the client’s age and gender, any specific precautions, family health history, and application of product. You will also need to instruct the client how often to use the product and what time of day is best.

What Questions Should You Ask on an Aromatherapy Client Intake Form?

Obviously, you will collect information such as name, date of birth, address, phone number, and email address. I usually have the client fill out the form, then during the interview ask questions about their responses to dig a little deeper into the symptoms and reasons for the existing condition.
Here are a few questions you will want to include on your aromatherapy client intake form:
- How would you describe your overall health?
- What are you hoping essential oils can do for your health?
- Do you have any chronic illnesses?
- If yes, what type of condition?
- How long have you been aware of this condition?
- What type of treatment (s) have you tried?
- Has this helped? List treatments that have been helpful.
- Which symptoms are most difficult for you?
- Do you have any acute conditions you would like to address?
- Please list any allergies:
- Are you pregnant or trying to become pregnant?
- Do you have epilepsy?
- Do you have high/low blood pressure?
- Which oils or aromas are you drawn to?
- Do any oils or aroma’s disturb you?
- Are you under the care of a physician? If so, please list the condition(s) you are being treated for:
- Please list any medications you are taking:
